Families and Communities Together (FCT) is a collaboration between Community Healthlink, LUK Inc., and Communities of Care that oversee three Wraparound programs operating in the Central MA area as part of the Commonwealth's Children's Behavioral Health Initiative (CBHI).
The FCT Wraparound programs seek to build on family strengths and available support systems to help children with serious emotional disturbance remain in or return to their communities.
The FCT staff work with a family to develop a Care Planning Team (CPT), which consists of the child, family, people invited by family, FCT staff, and appropriate state agency and school staff.
The Care Planning Team develops an Individual Care Plan (ICP) that focuses on the child's and family's needs and proposes ways to improve the child's well-being at home, at school, and in the community.
