Overview

PBHCI-The primary and behavioral health care integration program (PBHCI) provides access to a primary care physician (PCP) in our outpatient clinic for our consumers with severe and persistent mental illness who are not currently connected to a PCP.  The PBHCI model includes nurse care managers who will coordinate care with the behavioral and primary care providers, perform wellness assessments, develop wellness treatment plans and develop and facilitate wellness activities.

Responsibilities

  • Complete medical and wellness assessments of clients in an integrated primary care/ behavioral health setting
  • Provide individual and group health and wellness education including but not limited to, medications, chronic disease management, stress reduction, nutrition, and other general health concerns
  • Work with primary care practitioners and behavioral health team
  • Coordination of client care as needed, with all health care providers, families, residential home staff and others
  • Monitor and administer medication as prescribed
  • Provide crisis and emergency assessment and intervention
  • Treatment planning and assessment
  • Document in the electronic health records according to CHL policies
  • Attend all required meetings and trainings
  • Stay current with current nursing practice
  • Perform related duties as assigned
  • Will require transporting Clients in personal vehicle

Qualifications

  • Current valid MA RN license 
  • Self-directed, innovative, flexible, and adaptability
  • Strong knowledge of Electronic Health Records
  • Experience with quality improvement process to improve client care and meet required health risk outcome measures
  • Excellent communication and documentation skills
  • 2-3 years medical nursing experience with behavioral health and substance abuse experience preferred
  • Interest and knowledge about health and wellness, especially regarding clients with medical and behavioral health issues
  • Work well with clients individually and in groups
  • Work well independently and with an integrated team
  • Desire to outreach clients served in the community
  • Current valid US-issued drivers license and ability to provide registered, inspected and insured automobile for work related purposes, including transporting clients in your own vehicle
  • Must be able to pass a CORI background check
