Overview

Community Healthlink’s CBFS structure is a clinically sophisticated treatment and support system built to reflect the values of recovery, and the vision of hope and self empowerment.  Persons served in this dynamic system will receive a full range of rehabilitation, support, housing, and supervision.  Our system is designed to be flexible and continuous and highly integrated with staff including clinicians, nurses, rehab specialists, vocational specialists, and peer specialists.   

Responsibilities

To provide counseling services to consumers with substance abuse and mental health issues. Facilitating groups, writing behavioral treatments plans individual counseling and aftercare planning and participating as a member of multi-disciplinary team. This a psychiatric rehabilitation model under community based flexible support.

 

  1. Conducts consumer’s evaluations including bio-psychosocial assessment, diagnostic formulation and initial treatment recommendations.
  2. Facilitate rehabilitation groups using standard group techniques. Meet with consumers for individual counseling to assist in attending treatment goals.
  3. Write treatment plans including consumer's strengths with behaviorally based goals and objectives. Document progress of goals and objectives in progress notes.
  4. Follow a person-centered model and perform person-centered planning (IAP's), working with severe and persistent mental illness and dually diagnosed clients.
  5.  Maintain productivity standard as set by supervisor.
  6. Attend all scheduled meetings on time and remain for the duration of the meeting.
  7. Participate in agency committees, or community education as assigned.
  8.  Be an active participant in the multi-disciplinary team meeting and case discussions.
  9. Recognize and seek consultation as needed. Develop a training plan to enhance professional growth.
  10. Refer consumers to appropriate resources.
  11. Follow all policies and procedures of the agency.
  12. Participate in utilization management, employ CQI principals and assist as needed in accreditation processes.
  13. Position may require moderate physical duties.

This position is productivity bonus eligible

Qualifications

  • Grade 7: Clinician III: Master's Degree in psychology, counseling or social work required.

  • License to practice independently is preferred.

  • Current valid US-issued drivers license and ability to provide registed and insured vehicle for work related purposes including transporting clients

  • Must be able to pass CORI background check
