Rehabilitation Specialist -Worcester Residential Assistance Program*

WRAP - Worcester Residential Assistance Program: Since 1994 WRAP has been providing services to thirty-five high-risk individuals who reside in their own apartments, some of which are subsidized by HUD. Supported services are funded through DMH and consist of psychological rehabilitation and relapse prevention. Staff is available twenty-four hours a day via pager. Residents are homeless upon admission and have resided in the People in Peril (PIP) Shelter, Oasis House, or other emergency housing. Referrals are made through DMH.



  • In-House programming: Responsible for promoting a rehabilitative and home-like environment.   Assess, identify and implement skills using a teaching curricula based on residents’ need areas.
  • Needs Assessment: Train and oversee counselors in assessing and monitoring residents’ skill levels. Ensure that documentation for treatment plans are maintained and up to date.
  • Quality Improvement: Evaluate and modify in-house groups and programming. Ensure philosophy, goals, and group log notes are written and implemented for each group. Identify needs and training for staff in order to improve job knowledge and function.
  • Treatment Planning: Assist counselors and residents in developing and implementing behavioral treatment plans. Meet with Coordinator and counselor to assess the appropriateness of treatment plans at least semi-annually
  • Groups: Teach staff to facilitate groups and complete group assessments. Co-facilitate groups based upon residents needs, assist with house meeting when possible, and coordinate recreational activities
  • Community Living: Work with residents to assist with activities of daily living and foster a sense of safety in the community which includes personal hygiene and self-care skills, money management and budgeting symptom management, relapse prevention, use of public transportation and transporting,    problem solving and conflict resolution, and other interventions as necessary.
  • Direct Service: May serve as a counselor for some residents as requested by coordinator (Reference counselor’s job description for detailed duties).
  • Daily Routine: When indicated, assist in meal preparation, monitor medications and changes, Doctor’s appointments, transportation needs. Work with residents to maintain the residence on a daily basis including chores, bedroom, upkeep, yard, and other housekeeping duties.
  • Routine Documentation: Prepare daily log entries, incident reports, and other paperwork necessary to insure quality standards are maintained.
  • Other Duties: Assess and identify other related duties that are needed in order to provide an empowering, educational, and advancement programming. May include transporting residents in personal car or program van.
  • Maintain Competencies: Human Rights, Dual Diagnosis, Risk Management, Infection Control, MAP, CPR, First Aid, agency and divisional policy and procedures as well as other required competencies
  • Must be able pass required trainings including Medication Administration Program (MAP), CPR and First Aid


  • Grade 5: Rehabilitation Specialist I: HSD/GED/HiSet required; Associates degree preferred 
  • Grade 6: Rehabilitation Specialist II: Bachelor's degree required. Degree in social work, psychology or related field preferred.
  • Demonstrate knowledge of the rehabilitative process including independent living, vocational and community living skills, and apply this process in 1:1 interventions and group settings.
  • Ability to assess community resources that would assist clients in overcoming specific impairments.
  • Display strong planning, organizing and problem solving skills; in order to establish goals, and delegate/coordinate week day and weekend activities.  
  • Current valid US-issued drivers license and ability to provide registered, inpected and insured automobile for work related purposes including transporting clients in your own vehicle
  • Effective verbal and written communication with counselors, peers, and other professionals.
  • Must be able to pass a CORI background check
