MyLink, a collaborative project with Hospital Emergency Departments and Primary Care Practices, is a coordinated response to care for individuals with multiple health and social needs who frequently turn to the Hospital Emergency Department for help.


Our purpose:

  • To work with ER doctors, nurses, social workers and care managers to reach out to individuals who frequently visit the ER and are in need of support.
  • To work with Primary Care Practices and Community Behavioral Health providers to facilitate connection and care coordination of individuals with complex health needs. 


  • To work with primary care practices and intensive health care management programs to identify and reach out to individuals with complex medical and behavior health conditions in order to coordinate care.
  • Responsible for clinical/care management supervision of staff assigned and of operations in accordance with the CHL standards and policies and procedures.
  • Ability to coordinate care for individuals with complex health and behavior health needs.
  • Clinical supervision of community support and outreach workers.
  •  Ability to effectively communicate and develop positive professional relationships with staff and co-workers and outside professionals.
  • Knowledge of and ability to work effectively with community based resources, referral networks, and public/private systems of care


  • Must have Human Services and/or experience in the substance abuse field.
  • Must be licensed as an RN, OT or LICSW or LMHC.
  • At least 3 years of experience providing service in the community.
  • Strong communication and listening skills
  • Comfortable working independently and within a group
  • Flexible yet organized
  • Must be able to pass a CORI background check
