Treatment Coordinator-High Intensity Supported Living Environment*

Community Healthlink’s CBFS has added a new intensive level of care for Supportive Living clients.  This high intensity comprehensive program will provide rehabilitative interventions and supports as well as clinical oversight to 8 residents who will live in their own apartments.  The program promotes and facilitates recovery, hope, and self empowerment.  Services include interventions and supports that manage psychiatric symptoms. Assistance will be given to clients to restore or maintain independent living in the community, restore or maintain daily living skills, promote wellness and the management of medical conditions and assist clients to use their current strengths and skills to undertake employment. Individual treatment plans are developed for each resident. Staff is available to residents 24 hours per day.  All residents are DMH referred.


  • Performs duties using recovery orientated principles.
  • Demonstrate person centered strength based communication and relationship with consumers.
  • Effective verbal and written communication with consumers, peers, supervisors and other professionals.
  • Ability to observe, document, teach, and respond to client mental, and physical health, as well as other needs of consumers.
  • Display strong planning, organizing, and problem solving skills, in order to coordinate crisis management needs.
  • Encourage and monitor resident’s participation in the treatment program as indicated on treatment plans, PSTP’s and Personal Recovery Plans.   


  • Treatment Planning: Complete Mental, Physical, and Psychosocial Assessments (MPPA) as assigned. Develop, implement and document, treatment plans, progress notes, and quarterly reviews.
  • Community Living: Work with consumers to assist with activities of daily living and foster a sense of safety in the community which includes personal hygiene and self-care skills, money management and budgeting, symptom management, relapse prevention, use of public transportation and transporting, problem solving and conflict resolution and other interventions as necessary.
  • Medications: Monitor medication, refills, medications changes, and maintain and comply with regulations for DPH MAP certification.
  • Routine Paperwork: Prepare daily log entries, incident reports, census, APR reports and other required paperwork to ensure the program meets all required standards.
  • Continuity of Care: Attend team meetings, rounds and case conferences, and collaborate with other providers for designated consumers.
  • Coordinate Referrals: Screen and evaluate referrals, ensure all required documentation is received, and engage consumer in to the program.
  • Treatment Contracts: Assist the Program Coordinator in the development and implementation of treatment contracts.
  • Case Coordination: Act as a liaison with other service providers by keeping them informed of clients’ mental and physical health.
  • Crisis Management: Coordinate appropriate level of care for consumers who are in crisis.
  • Other Duties: Other related duties as assigned in order to provide quality rehabilitation services. Will include transporting residents in personal car and/or program vehicle.
  • Maintain Competencies: Human Rights, Dual Diagnosis, Infection Control, CPR, First Aid, MAP Certification, agency and divisional policy and procedures as well as other required competencies. 
  • Oversee program in absence of Program Coordinator.


  • Master’s Degree in social work or related human services field preferred; or Bachelor’s degree in a related field plus 2 years experience.
  • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle
  • Must be able pass required trainings including Medication Administration Program (MAP), CPR and First Aid
  • Must be able to pass a CORI background check
