The Program for Assertive community treatment, or PACT, is an intensive and highly integrated approach for community mental health service delivery that serves individuals who struggle with serious and persistent mental illness and whose symptoms result in serious functioning difficulties in several major areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.
Unlike other community-based programs, PACT is not a linkage case-management program that connects individual’s services. Rather, it provides highly individualized services directly to consumers. PACT is organized in a team model that includes doctors of psychiatry, mental health therapists, nurses, and vocational specialists who work together to create individualized plans for clients and provide the care needed to meet the goals of these plans. Services are provided in clients’ own homes and communities and are provided 24 hours a day, seven days a week, and 365 days a year.
Community Healthlink operates two PACT teams in the Worcester County area. One covers the North Worcester County area (PACT III) and the other cover the Central Worcester County area (PACT). This program is closed to only DMH referrals.
