CHL's PACT (Program for Assertive Community Treatment) Program is a DMH closed referral program that provides intensive treatment and support services for adults who have been diagnosed with persistent chronic mental illnesses. The program utilizes a multidisciplinary team that includes a psychiatrist, registered nurses, social workers and vocational specialists whose goals are to support clients to reduce hospitalizations and to restore and enhance the emotional well-being and level of social functioning of clients.  The program also works closely with clients to find housing and to provide vocational training and employment opportunities.  The PACT program works with individuals residing in Worcester.



Clinician sought for busy community outreach team supporting adults with severe and persistent mental illness.  The Program for Assertive Community Treatment (PACT) is a community outreach, multidisciplinary team, working with individuals in the community to work towards recovery based goals.


The Clinician on the team would have responsibilities which include but are not exclusive to:


  • Providing therapeutic interventions in the form of person-centered, individual therapy to assist person served in actualizing their goals.
  • Working as part of a multi-disciplinary team.
  • Completing documentation in a timely manner.
  • Assisting clients in learning skills/coping strategies that support recovery.
  • Familiar with evidence based clinical practices (i.e. harm reduction, motivational interviewing, CBT, DBT, etc.).
  • Assisting individuals with activities of daily living, housing and vocational support.
  • Liason with medical providers and other community supports.



  • Grade 7: Clinician III: Master's degree in Social Work or Master's in Counseling required, Must be licensed or license eligible
  • Grade 8: Clinician IV: Master's degree in social work or a related field and an LICSW
  • Minimum of 4 years experience working with adults with mental illness.
  • Strong organizational skills and the ability to work cooperatively in a team environment a must.
  • Must be able to pass a CORI background check
  • Current valid US-issued drivers license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle
