Overview

CHL's Leominster Counseling Center, located in Leominster, MA, provides provides outpatient mental health services to adults living with with mental illness and/or substance abuse or substance abuse recovery. Available services include: psychiatric services; intensive outpatient therapy; individual, family and group psychotherapy; medication management, and crisis management. The center's experienced multi-disciplinary treatment team includes psychiatrists, psychologists, nurses, social workers, and mental health counselors.

Responsibilities

  • Provides direct clinical assessment and treatment of adults in an outpatient behavioral health setting.
  • Monitors medication in consultation with psychiatrist.
  • Administers medication as prescribed by psychiatrist.
  • Able to address other medication and health related issues including medication refills, medication education for clients, family members, program staff and others involved with client care internally and externally.
  • Provides crisis and emergency assessment and interventions.
  • Treatment planning and assessment.
  • Documentation in electronic health record (EHR), maintaining client record according to CHL policy.
  • Work with an interdisciplinary team.
  • Care coordination with integrated primary care with regards to medical and psychiatric health.
  • Attends all required administrative staff meetings.
  • Stay current with nursing practice.
  • Perform related duties as assigned.

 

Qualifications

  • Grade 9A- Valid and current LPN license in Massachusetts required.
  • Grade 9B-Acute Care RN: Current RN license in Massachusetts
  • BSN preferred
  • Minumum 2-3 years clinical nursing experience in adult psychiatric and substance abuse clinical setting
  • Excellent communication skills
  • Excellent assessment skills
  • Knowledge of electronic health record (EHR) and computer skills
  • Excellent documentation skills
  • Works well independently and with an interdisciplinary team
  • Works well with clients individually and groups
  • Current CPR certification
  • Bilingual preferred
  • Must be able to pass a CORI background check
