Individuals receiving services in Health Home Management are able to access primary and behavioral health care management in an integrated model. Every client receives care coordination, and care support for practical and social needs. All integration of services is managed through the health home management model.


Rather than being a physical place, health homes are a strategy for helping individuals with chronic conditions manage those conditions better.

An eligible individual—for example, a person with diabetes and a mental illness—works with a team of health care professionals to provide the following:

  • Management and coordination of all the services the person receives from multiple providers.
  • Help with transitions from one kind of setting to another.
  • Support to both the individual and their family members.
  • Offer referrals to community and social support services.


  • Ensures coordination between Program Director and department staff by maintaining calendars, professional meetings, vacations, conferences and travel.
  • Maintains office supply inventory by checking stock, anticipating needs, placing and expediting orders, and verifying receipt.
  • Keeps office equipment operating by following operating instructions, trouble shooting breakdowns, arranging for preventive maintenance, calling for repairs and orders and ensures follow through.
  • Initiating maintenance requests for office needs
  • Receiving, tracking and submitting invoices to accounts payable to ensure payment to outside vendors.
  • Acting as a back up to office staff by screening incoming telephone calls, recording messages, scheduling, arranging referrals to other health care providers, and scheduling appointments for consultations, x-rays, lab tests, physical therapy, MRI's, CT scans, etc.
  • Assists with preparing letters and communication to patients and staff
  • Attend office meetings and assist Program Director in taking minutes to send to all appropriate staff for proper communication in the event of absences.
  • Coordinates the offices administrative activities and may help Program Director with reports for quality improvement projects.


  • Grade 4: Senior Secretary- HSD/HiSet/GED required

  • Minimum of 1 year experience working as a secretary or receptionist

  • Must have excellent communication skills

  • Ability to work in a team setting as well as independently

  • Proficient in Microsoft Word, Excel and Power Point 

  • Must be able to pass a CORI background check
