Community Healthlink’s CBFS is an intensive level of care for Supportive Living clients.  This high intensity comprehensive program will provide rehabilitative  interventions and supports as well as clinical oversight  to 8 residents who will live in their own apartments.  The program promotes and facilitates recovery, hope, and self empowerment.  Services include interventions and supports that manage psychiatric symptoms. Assistance will be given to clients to restore or maintain independent living in the community, restore or maintain daily living skills, promote wellness and the management of medical conditions and assist clients to use their current strengths and skills to undertake employment. Individual treatment plans are developed for each resident. Staff is available to residents 24 hours per day.  All residents are DMH referred.






  • Performs duties using recovery orientated principles.     
  • Ability to hire, train, supervise and evaluates program staff and ensures required competencies are maintained. 
  • Oversight of clinical operations including behavioral and rehabilitative treatment planning for mental health and substance abuse.
  • Demonstrate effective communication with consumers, staff, peers, and other professionals.
  • Operate program in an efficient and effective manner ensuring compliance with BSAS, DPH, HUD, DMH and MAP regulatory requirements.
  • Demonstrate person centered strength based communication and relationship with consumers. 


  • Quality Management: Provide oversight of Respite Beds Program.  Act as Program Coordinator during his/her absence.
  • Supervision and Training: Orient and train new staff, as designated, to daily operations of the program and Division procedures. Ensure new staff are oriented and trained as required by CHL.  Provide ongoing supervision to program staff and peers. 
  • Census: Ensure accuracy and timeliness of consumer calendar, admission and discharge forms, special billing, and/or Service Activity Logs.
  • Program Management: Plan, Implement and continuously evaluate rehabilitative and support services through assessments, treatment planning to meet individualized needs, and development of peer support groups. Schedule with peers individual times to meet with consumers.
  • Personnel: Take personnel action in regard to disciplinary procedures.
  • Facilities: Identify problem areas and coordinate work for upkeep of site. Insure day-to-day upkeep of residence and workplace. Work with consumers and staff to maintain the residence on a daily basis including chores, bedroom upkeep, yard and other housekeeping duties.   
  • Liaison: Serve as a liaison among other departments and with other agencies in order to facilitate a continuum of care.
  • Other Duties: Perform other related professional duties required in order to provide quality clinical programming. Transportation of consumers may be required in personal car or program vehicle.  


  • Master’s degree in social work or related human services field or a Bachelor’s Degree in a related field plus 1 year experience.
  • Will consider several years experience in lieu of college degree.
  • Current valid US-issued drivers license and ability to provide a registered, inspected and insured automobile for work relted purposes including transporting clients in your own vehicle
  • Must have supervisory experience 
  • Must be able to pass a CORI background check
