Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) RN-Care Manager in Worcester.


Community Healthlink’s Behavioral Health Community Partner (BHCP) program is a new, innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) and or a Managed Care Organization (MCO). 


Multidisciplinary BHCP teams will play a key role in providing high quality integrated care; functioning as a bridge between clients, behavioral health providers and medical providers. Team members will use their expertise to support individuals to navigate health and social systems in order to help members address social determinants health, and improve overall quality of life and health.


The BHCP RN Care Manager is a core member of the collaborative care team which includes the enrollee’s medical providers and behavioral health providers, and is responsible for supporting and coordinating the mental and physical health care of enrollees assigned. The RN Care Manager works collaboratively with the interdisciplinary team to ensure engaged enrollee’s needs are met and care delivery is coordinated across the continuum.

  • The RN Care Manager serves as a liaison between the hospital, primary care practices and community agencies or facilities for the exchange of clinical and referral information.
  • Perform a Comprehensive Assessment and participate in Care Planning.
  • Conduct medication reconciliation, verify medication orders are appropriate, check for discrepancies, ensure that enrollees understand medication administration and adhere to medication regimen.
  • Provide Health education and Wellness coaching to help promote healthy activities, manage chronic medical conditions and reduce high risk behaviors.
  • Engage enrollees in setting wellness goals in Person Centered Care Plan and promote healthy activities such as smoking cessation and weight loss.
  • Knowledge of and ability to work effectively with community based resources, referral networks, and public/private systems of care.


  • Grade 9: Community RN: Current valid MA RN license
  • Must have a minimum of one year's experience
  • Current valid US-issued drivers license and ability to provide a registered, inspected and insured automobile for work related purposes including travel between work sites
  • Must be able to pass a CORI background check
