Community Healthlink’s Behavioral Health Community Partner (BHCP) program is a new, innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) and or a Managed Care Organization (MCO).
Multidisciplinary BHCP teams will play a key role in providing high quality integrated care; functioning as a bridge between clients, behavioral health providers and medical providers. Team members will use their expertise to support individuals to navigate health and social systems in order to help members address social determinants health, and improve overall quality of life and health.
The BHCP RN Care Manager Supervisor will supervise RNs and LPNs who are part of the BHCP Care Coordination Teams. The RN Supervisor will help with implementing this exciting, innovative service for MassHealth members with complex medical, behavioral health and social needs. The RN supervisor will have experience in chronic disease management and be able to provide training in wellness coaching, medication reconciliation and person-centered care planning. The RN Supervisor will help develop procedures and work-flows that are highly collaboration, instill hope, and most importantly, center on the Person’s strengths.
