Community Based Flexible Supports (CBFS) is a comprehensive, clinically sophisticated treatment and support system built to reflect the values of recovery, and the vision of hope and self empowerment.  Our CBFS system is dynamic, flexible and provides a full range of rehabilitation,  housing option, employment support, and wellness management.  Our system is designed to be flexible and continuous and highly integrated with staff including clinicians, nurses, rehab specialists, vocational specialists, and peer specialists


General Duties:


  • Perform duties using recovery orientated principles.
  • Demonstrate person-centered, strength-based communication and relationship with consumers. 
  • Effective verbal and written communication with all personelle. 
  • Ability to observe, document, teach, and respond to client mental, and physical health, as well as other needs of consumers. 
  • Display strong planning, organizing, and problem solving skills, in order to coordinate crisis management needs.
  • Encourage and monitor resident’s participation in the treatment program as indicated on treatment plans, IAP’s and Personal Recovery Plans. 


Specific Duties

  • Complete Mental, Physical, and Psychosocial Assessments (MPPA) as assigned. Develop, implement and document, treatment plans, progress notes, and quarterly reviews.
  • Oversee interventions to assist persons with activities of daily living and foster a sense of safety in the community which includes personal hygiene and self-care skills, money management and budgeting, symptom management, relapse prevention, use of public transportation and transporting, problem solving and conflict resolution and other interventions as necessary.
  • Monitor medication, refills, medications changes, and maintain and comply with regulations for DPH MAP certification.
  • Prepare daily log entries, incident reports, census, APR reports and other required paperwork to ensure the program meets all required standards. 
  • Attend team meetings, rounds and case conferences, and collaborate with other providers for designated consumers. 
  • Screen and evaluate referrals, ensure all required documentation is received, and engage consumer in to the program.
  • Assist the Program Coordinator in the development and implementation of treatment contracts.
  • Act as a liaison with other service providers by keeping them informed of clients’ mental and physical health. 
  • Coordinate appropriate level of care for consumers who are in crisis.
  • Perform other job related duties as assigned in order to provide quality rehabilitation services.
  • Will include transporting residents in personal car and/or program vehicle.
  • Human Rights, Dual Diagnosis, Infection Control, CPR, First Aid, MAP Certification, agency and divisional policy and procedures as well as other required competencies. 
  • Oversee program in absence of Program Coordinator.


  • Master’s Degree in social work or counseling
  • Must be licensed or license eligible with license by 6/30/18 - LCSW, LICSW, LMHC
  • Current valid US-issued driver’s license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle or a company vehicle
  • Must be able to pass a CORI background check
