Community Healthlink's (CHL) Respite program provides 11 respite beds on its main Worcester campus for the DMH Worcester service system.


The program provides temporary housing and support services for clients who are in transition from one living arrangement to another-sometimes coming directly from Worcester State Hospital or a community inpatient unit.


The Respite program offers a community-based Crisis Stabilization Unit and medical respite is provided for homeless clients who are recovering from an illness or injury.


  • Treatment Planning: Provide appropriate case management services outlined in the treatment plan as assigned under supervision of Program Coordinator; develops and facilitates implementation of after-care plan.
  • Outreach and Intake: Conduct outreach activities at appropriate referral sites, Work closely with other providers to receive referrals, conduct admissions interviews and initial assessments for review by Program Coordinator and Clinical Team.
  • Advocacy: Ensures residents receive appropriate entitlements, assists residents to resolve outstanding legal issues, ensures documentation of information, and provides transportation when necessary.
  • Medications: assist with client’s attendance at appointments and compliance with medication protocols
  • Groups: Lead at least one skill development or recreational group per week as assigned.
  • Continuity of Care: Attend teams, case conferences, and collaborate with other providers for designated residents.  Provide outreach as designated to community-based residents.
  • Routine Paperwork: Prepare daily log entries, incident reports, SALs and other required paperwork to ensure the program meets Residential Division Standards.
  • Other Duties: Perform other related job duties as assigned in order to provide quality rehabilitation services. Will include transporting residents in personal car and/or program van.


  • Grade 5: Case Manager I: HSD/GED/HiSet required; Associate’s Degree preferred and relevant experience.
  • Grade 5: Case Manager II: Bachelor’s Degree in social work or related substance abuse field.
  • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in a person vehicle 
  • Previous experience working with clients in human services preferred.
  • Proven ability to work within team setting and to act as a self-starter.
  • Must be committed to providing quality care.
