Families and Communities Together (FCT) is a collaboration between Community Healthlink, LUK Inc., and Communities of Care that oversee three Wraparound programs operating in the Central MA area as part of the Commonwealth's Children's Behavioral Health Initiative(CBHI).
The FCT Wraparound programs seek to build on family strengths and available support systems to help children with serious emotional disturbance remain in or return to their communities.
The FCT staff work with a family to develop a Care Planning Team (CPT), which consists of the child, family, people invited by family, FCT staff, and appropriate state agency and school staff.
The Care Planning Team develops an Individual Care Plan (ICP) that focuses on the child's and family's needs and proposes ways to improve the child's well-being at home, at school, and in the community.
Under the direction of a Supervisor of Intensive Care Coordination, provides Intensive Care Coordination (ICC) to youth with serious emotional disturbance (SED) and their families. The Care Coordinator works in partnership with a Family Partner to help develop, coordinate, and implement services and supports for enrolled youth and families using the Wraparound Service Planning process as defined by the Massachusetts Children’s Behavioral Health Initiative (CBHI). This process includes: successful engagement of the youth and family, assessment of needs, values, and strengths, building a unique and individualized Care Planning Team, facilitating Care Planning Team meetings that result in the development of a Care Plan, and monitoring, coordinating, and helping to implement that plan with families and youth as full partners in the process.
