    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Nurse Practitioner in Worcester. 


    Community Healthlink’s Homeless Outreach and Advocacy Project (HOAP) provides direct services to individuals who are homeless.  Services include outpatient primary health care, behavioral health care (individual and group mental health and addiction treatment), and outreach and case management services.  Working together, our integrated, multi-disciplinary treatment teams address physical and mental health needs, substance use disorders, and identified social determinants of health by providing access to a wide-variety of patient-centered, trauma-informed treatment and services. CHL HOAP is a Federally Qualified Health Center (FQHC) Health Care for the Homeless (HCH) program.

    • Provides medical assessment and completes diagnostic exams for HOAP patients.
    • Orders diagnostic tests and referrals as indicated.
    • Develops comprehensive medical care plans.
    • Prescribes medications as indicated and monitors effectives.
    • Works as part of a multi-disciplinary team.
    • May be asked to deliver care at outreach locations.
    • Works in a scheduled and urgent care capacity.
    • Completes all documentation in a timely manner.
    • Current in best practices for primary care.
    • Works well with special populations.


    • Masters degree required
    • Valid Nurse Practitioner License.
    • Minimum of 3 years prior experience working in a primary care or medical capacity as an NP
    • Knowledgeable about issues of homelessness and special health care needs of this population.
    • Works well with a multi-disciplinary model.
    • Familiar with diagnostic equipment and testing.
    • Comfortable using computers.
    • Strong documentation skills.
    • Must be able to pass a CORI background check
