Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Case Manager in Leominster, MA (some travel to Worcester required).
Community Healthlink’s Community Support Program (CSP) provides short term, intensive outreach and case management services to support individuals at risk of repeated psychiatric hospitalizations and/or inpatient substance abuse treatment admissions. CSP services are flexible and based on individual needs, but focus primarily on helping clients overcome challenges with establishing and maintaining access to health care, community based resources, and social/peer supports.
CSP case managers meet with referred clients in a variety of community settings, complete intake assessments and person centered service plans, then assist clients with achieving the goals identified in those plans. CSP duties include researching community based resources and assisting clients to connect with these resources by helping with things such as short term transportation, advocacy, encouragement, skills building, coaching, and completing paperwork.
