    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Specialty Site Manager in Worcester.

     

    The High Intensive Structured Living Environment (HISLE) program is an intensive level of care for persons with mental health and substance use issues. This comprehensive program will provide rehabilitative interventions and supports as well as clinical oversight to 8 residents who will live in their own apartments within the same building.

    The Intensive Clinical Specialty Site Manager will oversee the provision of all clinical services for Persons at high risk and/or are difficult to engage and who have a serious mental illness with or without a co-occurring substance use history. They will provide clinical supervision of Licensed staff and other staff as required and assist with selecting intervention.

    Responsibilities

    • Assists with selecting invention strategies for Persons at high risk and/or difficult to engage.
    • Supervise, support, and/or mentor Licensed staff members and other staff as required.
    • Facilitate and participate in team meetings to coordinate enrollment of new Persons and/or assist with transition or disenrollment/discharge from services.
    • Work with other licensed practitioners to complete an initial assessment or a comprehensive assessment to determine individual clinical and rehab needs. 
    • Work with other licensed practitioners to develop Treatment Plans and Clinical intervention.
    • Effectively communicate with all members of the team to ensure that services are delivered in a collaborative, clinically integrated, culturally integrated, culturally responsive and recovery orientated manner.
    • Ensure that services are person centered and truly encourages the persons participation in her/her own recovery.
    • Delivers rapid response to Persons emergency in clinical needs, including but not limited to Symptom Management, de-escalation strategies and one to one assistance. Will work with staff in this area. 
    • Daily Rounds with all team members present.
    • Will provide transportation to persons as needed. 
    • May include transporting in individuals in personal or program vehicle. 
    • Perform other related job duties.

    Qualifications

    • Masters degree required
    • Must have be clinically licensed or be license eligible (LICSW, LCSW or LMHC)
    • At least two years of providing clinical supervision.
    • Knowledge and experience with trauma informed care.
    • Knowledge and experience with psychosocial rehabilitation model
    • Current valid US-issued drivers license and a registered, inspected and insured automobile for work related purposes including transporting clients in a personal vehicle or company van
