Community Healthlink’s Homeless Outreach and Advocacy Project (HOAP) provides direct services to individuals who are homeless. Services include outpatient primary health care, behavioral health care (individual and group mental health and addiction treatment), and outreach and case management services. Working together, our integrated, multi-disciplinary treatment teams address physical and mental health needs, substance use disorders, and identified social determinants of health by providing access to a wide-variety of patient-centered, trauma-informed treatment and services. CHL HOAP is a Federally Qualified Health Center (FQHC) Health Care for the Homeless (HCH) program.
The HOAP Clinic has provided integrated primary and behavioral health care and substance use treatment for more than 30 years. Our highly-qualified, trauma-informed providers and clinicians utilize case managers, care consultants, and nurse care managers to provide a person-centered team approach to meet the needs of each patient through individual and group treatment.
Community Healthlink’s Homeless Outreach and Advocacy Program Senior Receptionists are responsible for the capturing of patients demographic information, scheduling appointments, referrals, insurance verification all while ensuring our patients receive the highest level of customer service.
