    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Community RN in Worcester.


    Community Healthlink’s Homeless Outreach and Advocacy Project (HOAP) provides direct services to individuals who are homeless.  Services include outpatient primary health care, behavioral health care (individual and group mental health and addiction treatment), and outreach and case management services.  Working together, our integrated, multi-disciplinary treatment teams address physical and mental health needs, substance use disorders, and identified social determinants of health by providing access to a wide-variety of patient-centered, trauma-informed treatment and services. CHL HOAP is a Federally Qualified Health Center (FQHC) Health Care for the Homeless (HCH) program.


    The HOAP Clinic has provided integrated primary and behavioral health care and substance use treatment for more than 30 years.  Our highly-qualified, trauma-informed providers and clinicians provide a person-centered team approach to meet the needs of each patient through individual and group treatment.  


    Community RN's provide   nursing   assessment,   treatment, education and health care service coordination to individuals  and families. Their focus is on promoting and maintaining an optimal level of health. The RN collaborates with all components of the team and other health care agencies to ensure coordinated, comprehensive services. Services will be provided at CHL and outreach locations

    • Complete medical and wellness assessments of clients in an integrated primary care/ behavioral health setting
    • Provide individual and group health and wellness education including but not limited to, medications, chronic disease management, stress reduction, nutrition, and other general health concerns
    • Work with primary care practitioners and behavioral health team
    • Coordination of client care as needed, with all health care providers, families, residential home staff and others
    • Monitor and administer medication as prescribed
    • Provide crisis and emergency assessment and interventions
    • Treatment planning and assessment
    • Document in the electronic health records according to CHL policies
    • Attend all required meetings and trainings
    • Stay current with current nursing practice
    • Perform related duties as assigned


    • Grade 9: Community RN: Must have a current valid MA RN license
    • Self-directed, innovative, flexible, and adaptability
    • Strong knowledge of Electronic Health Records
    • Experience with quality improvement process to improve client care and meet required health risk outcome measures
    • Excellent communication and documentation skills
    • Interest and knowledge about health and wellness, especially regarding clients with medical and behavioral health issues
    • Work well with clients individually and in groups
    • Work well independently and with an integrated team
    • Desire to outreach clients served in the community
    • Current valid US-issued drivers license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in a personal vehicle
    • Must be able to pass a CORI background check
