    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Residential Counselor at our Worcester site.

     

    This residential program is bilingual/bi-cultural and serves 8 adult clients with persistent mental illness whose primary language is Spanish. This program is staffed 24/7. The Windsor Street program prides itself on fostering a family-like atmosphere, by providing a nurturing, safe, supportive and respectful milieu.   A dedicated team of staff work collaboratively with providers, providing education and support to each individual to ensure that they meet their individual goal.

     

    The program provides support such as medication management, symptom management, relapse prevention, transportation to appointments, healthy meal preparation, as well as staff lead a skills or recreational group weekly.   Staff attend team meetings and collaborate with providers, following through working with individuals on their specific needs, including symptom management, medication monitoring, relapse prevention. This position provides an opportunity to join a team of dedicated staff who provide ongoing support and services to individuals with persistent mental illness.

     

    Duties of a Residential Counselor consist of documentation, facilitating in house groups, one on one sessions with clients and assist with activities of daily living such as personal hygiene and self-care skills, money management and budgeting, symptom management, relapse prevention, use of public transportation and transporting, problem solving and conflict resolution, and other interventions as necessary. Residential Counselors will also transport clients when needed to outside AA/NA meetings or any appointments needing staff support.

    Responsibilities

     

    • Serve as residential counselor for specified number of residents as requested by supervisor.
    • Develop, implement and document treatment needs assessments, treatment plan, and progress reviews as part of continuous quality improvement activities.
    • Work with residents to assist with activities of daily living and foster a sense of safety in the community which includes personal hygiene and self-care skills, money management and budgeting, symptom management, relapse prevention, use of public transportation and transporting, problem solving and conflict resolution, and other interventions as necessary.
    • Ensure healthful menu planning, food shopping, and meal preparation through coordination and assistance.
    • Monitor residents’ medication, refills, documentation of medications and changes, assist with attending appointments, and safe storage.
    • Lead two or more skill development or recreational group (s) per week as assigned.
    • Attend teams and case conferences, and collaborate with other providers for designated residents. Provide outreach as designated to community-based residents.
    • Prepare daily log entries, incident reports and other required paperwork to ensure the program meets the standards.
    • Work with residents to maintain the residence on a daily basis including chores, bedroom upkeep, yard and other light housekeeping duties.
    • Perform other related job duties
    • May include transporting residents in personal car or program vehicle
    • Physical assistance, close supervision, and other such duties.
    • Must be able pass required trainings including Medication Administration Program (MAP), CPR and First Aid within first 5 months of employment

    Qualifications

    • Grade 5: Residential Counselor I: - HSD/GED or HiSet required, Associates degree preferred.
    • Grade 6: Residential Counselor II:- Bachelor's degree required; and coursework in social work, counseling, psychology or related field preferred.
    • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in a personal vehicle or a company van
    • Desire to serve others required.
    • Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives required.
    • Preference will be given to those with experience in social services and/or working with program's population.
    • Bilingual in English and Spanish required.
    • Must be able to pass a CORI background check
