Community Healthlink’s Homeless Outreach and Advocacy Project (HOAP) provides direct services to individuals who are homeless. Services include outpatient primary health care, behavioral health care (individual and group mental health and addiction treatment), and outreach and case management services. Working together, our integrated, multi-disciplinary treatment teams address physical and mental health needs, substance use disorders, and identified social determinants of health by providing access to a wide-variety of patient-centered, trauma-informed treatment and services. CHL HOAP is a Federally Qualified Health Center (FQHC) Health Care for the Homeless (HCH) program.
The HOAP Clinic has provided integrated primary and behavioral health care and substance use treatment for more than 30 years. Our highly-qualified, trauma-informed providers and clinicians utilize case managers, care consultants, and nurse care managers to provide a person-centered team approach to meet the needs of each patient through individual and group treatment.
Clinicians complete assessments and treatment plans, and offer individual and group therapy through outreach/home and in-office settings. Our integrated teams provide a low-barrier approach to quality interventions including both mental health and substance use disorder treatment. Supervision for licensure is available.

Grade 7- Clinician III: Masters degree in a clinical course of study in behavioral health (social work, psychology, marriage and family therapy, counseling or related field) that meet the academic requirements for independent licensure in psychology, social work, marriage and family therapy, or mental health counseling in the Commonwealth of Massachusetts, and the individual is licensed as a licensed Clinical Social Worker (LCSW), a licensed Marriage and Family Therapist (LMFT)
Grade 8- Clinician IV: Masters degree in a clinical course of study (social work, psychology, counseling or related field) and LICSW licensure or licensed Psychologist
