    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (40 hrs/wk) Advanced Practice Psychiatric RN in Leominster.


    The Leominster Adult Outpatient Center provides strength-based individual and group therapy to adults, and couples age 18 and over. We offer integrated, whole person care with primary care, a wellness program, and behavioral health. The Leominster Counseling Center also offers specialized substance use disorder treatment. Our office based opioid treatment offers buprenorphine and vivitrol treatment and we have a Second Offender Alcohol Education program.


    The APRN meets with clients for the purpose of medication evaluation, assessment and treatment planning. They consult with other members of the client’s treatment team; primary care, behavioral health clinician and outside providers such as DMH. The APRN may work one 4 hour shift in the Open Access Program and attends a monthly meeting with other providers and the Chief Medical Officer.

    • Provides assessment, diagnosis, medication consultation, and follow-up services for individual adults with severe and persistent mental illness, chronic substance use conditions, and co-occurring conditions, including individuals with co-occurring developmental disabilities. In collaboration with the clinical team this includes strengths, goals, need areas and additional consultation and collaboration as needed for treatment and safety.
    • Works closely with a psychiatric supervisor employed by UMass Memorial Healthcare through Community Healthlink.
    • Provides services at the counseling centers in Leominster, Fitchburg, and Gardne.
    • Works collaboratively as a member of patient-centered treatment teams, including the Leominster Counseling and Wellness Center’s multidisciplinary Open Access teams to coordinate the delivery of clinical and medical services.
    • Maintains all documentation for services provided, with review of information from other providers in the EMR.
    • Attends relevant training activities.
    • Regularly attends required program meetings.
    • Prepares and maintains confidential client documentation, service records, service activity logs (SALS.)
    • Performs other related job duties as required.

    • Licensure as a Psychiatric Clinical Nurse Specialist, Psychiatric Nurse Practitioner or Physician's Assistant by the Massachusetts Board of Registration in Nursing
    • Current DPH and DEA certificates
    • Experience working with adults with severe and persistent mental illness, substance use disorders, developmental disabilities, and co-occurring developmental disabilities and/or medical conditions
    • Meets all qualifications for the advanced practice of psychiatric nursing with prescribing privileges
    • Must be able to pass a CORI background check
