    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (40 hrs/wk) Family Partner at our Leominster site.

     

    Families and Communities Together (FCT) seeks to build on family strengths and available support systems to help children with serious emotional disturbance remain in or return to their communities.  FCT staff works with the family, child, appropriate state agencies and school staff to develop strategies that help improve child and family functioning.

     

    Sharing experiences from their own journey as a caregiver, the Family Partner works with parents to improve the capacity of the parent /caregiver to parent a youth who lives with Serious Emotional or Behavioral Disturbance. Through a mentoring relationship, a Family Partner will improve the parent’s functioning to support the youth in the community or to assist the youth in returning to the community.

    Responsibilities

    • Demonstrates a basic, functional knowledge of and endorses the system of care values:  Child Centered and Family Driven, Strength-Based, Culturally Responsive, Collaborative and Integrated, and Continuous Quality Improvement.
    • Is able to engage youth and their families in a mentoring relationship, by sharing their own journey, helping to model collaborative interactions between the family and the variety of relevant individuals in the larger system of support (extended family, friends, other providers, school personnel, and/or state agency representatives).
    • Demonstrated ability to effectively communicate with youth, families, colleagues, and community members in a manner that is sensitive to their racial/ethic/cultural/class backgrounds.
    • Demonstrates clear and consistent boundaries.  

     

    Essential Duties and Responsibilities:  

     

    • Assist in the creation and maintenance of a positive and collaborative working environment where all employees of CHL and enrolled youth and their families feel welcome, supported, and comfortable.
    • All activities described here will be delivered at the offices, in the community, or in the family’s home depending on the preferences of the enrolled youth and family.
    • Works to build strong, functional working relationships with enrolled youth and their families
    • Develop and implement outreach strategies when necessary to enable parents/caregivers of enrolled child/youth to participate fully in the Wraparound Process (if enrolled in ICC).
    • Assists the clinical hub provider (Care Coordinator, In Home Therapist, or Outpatient Provider) to develop with each youth and family a Care Planning Team composed of family members, their natural supports, and, if applicable, state agency representatives, school personnel, providers, and other significant persons.
    • Serves as a mentor to parents/caregivers to improve their ability to effectively advocate for their youth and navigate the child/family serving human services system, including the family’s active participation in the Wraparound process itself.
    • Works closely with the clinical hub provider, the family, and the Care Planning Team to monitor the Care Plan, and serves as an advocate/liaison for the family when appropriate to access new or different services.
    • Participates as appropriate in all meetings, training, evaluation/research, supervision, quality management, and related activities as may be directed by the Family Partner Supervisor.
    • Prepares all necessary paperwork such as contact notes, daily service logs, etc.
    • Follow all policies and procedures of the agency.
    • Work assigned hours, which may include some weekend and evening hours.
    • Perform other job related duties as assigned by supervisor.

    This position is productivity bonus eligible

    Qualifications

    • Current valid US-issued drivers license and ability to provide a registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle 
    • Must be able to pass a CORI background check
    • Experience as a caregiver of a youth with special needs—preferably a youth with mental health needs
    • Experience in navigating and working with any of the child/family serving systems or teaching other parents/caregivers to navigate such systems
    • Grade 5: Family Partner I: Associates degree in a human services related field from an accredited school and one (1) year of experience working with youth/families; or High School diploma, HiSet or GED and a minimum of two (2) years experience working with youth/families
    • Grade 6: Family Partner II: Bachelor’s degree in a human services related field from an accredited university or college and one (1) year of experience working with youth/families.
