    Come join a dedicated team that provides homelessness prevention services to tenants with disabilities. Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Case Manager in Worcester.

     

    The Tenancy Preservation Program (TPP) provides a clinical presence in the Central MA Division of the Housing Courts and short-term intensive case management to eligible households whose tenancy problems are related to a disability.  Our primary responsibility is to the Housing Court, providing professional assessment and recommendations regarding the disability-related causes of the tenancy problems and recommendations for service plans and other reasonable accommodations to address the tenancy problems. 

     

    Case Managers at TPP "Upstream" are responsible for providing short term intensive case management for eligible households pre-NTQ, providing intervention prior to court action. TPP case managers also provide consultation to tenants, landlords, and service providers on tenancy problems, the housing court process, and referral information for those deemed ineligible for the program.

    Responsibilities

    • Assess needs of individuals with disabilities who are at risk of losing their housing due to disability related issues.
    • Develop service plans to address issues effecting tenancy and complete referrals for on-going services.
    • Complete program evaluation documentation, monthly reports, reports for housing court and client specific case records and in quality improvement activities.
    • Participate in supervision, meetings and trainings.
    • Works closely with landlords, service providers, and housing court personnel
    • Ensure timely completion of all reports and paperwork.
    • Able to work independently and keep supervisor informed of service specific issues.
    • Familiar with skills necessary for maintaining independent living and understands how to access resources to support independent living.
    • Participate in other agency committees/work groups as needed.
    • Maintains knowledge base of services to assist in prevention of homelessness

     

    Qualifications

     

    • Grade 5: Case Manager I- HSD/HiSet/GED or Associates degree required
    • Grade 6: Case Manager II-Bachelors degree required; degree in psychology, social work, human services or related field preferred
    • Ability to effectively communicate and develop positive professional relationships with clients, staff and co-workers and outside stakeholders.
    • Strong clinical background and ability to translate clinical theory to real life application.
    • Able to work independently & Complete all work in an accurate, thorough and timely manner.
    • Have basic computer skills.
    • Strong verbal and written communication skills.
    • Functions well in formal work environment of housing court, local housing authorities and legal services
    • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in a personal vehicle
    • Desire to serve others required
    • Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives
    • Must be able to pass a CORI background check
