    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a part-time (10 hrs/wk) Nurse Educator at our Leominster site.


    Families and Communities Together (FCT) seeks to build on family strengths and available support systems to help children with serious emotional disturbance remain in or return to their communities.  FCT staff works with the family, child, appropriate state agencies and school staff to develop strategies that help improve child and family functioning.


    The nurse educator identifies and/or develop educational materials and assists with training FCT staff about a variety of wellness and health issues which include asthma, childhood obesity, Type I Diabetes, metabolic disorders, screen time, poly-pharmacy, and the role of good nutrition, exercise and sleep in improving health. 



    • Assist FCT staff with integration of wellness goals into the family's Individualized Care Plan.
    • Consult with FCT staff about how to collaborate with pediatric practices and family practices to address the particular health concerns of their patients.
    • Outreach to Pediatric and Family Practices to provide information about CSA services


    • Graduate of an accredited school of nursing with current license as Registered Nurse in the state of MA.
    • Experience working with youth and families.
    • Current valid US-issued drivers license and registered, inspected and insured automobile for work related purposes including travel between work sites
    • Must be able to pass a CORI background check
