    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Case Manager in Worcester.

     

    Behavioral Health Addiction Urgent Care (BHAUC) is a center for individuals suffering from substance use disorder and mental illness. The program is a gateway to getting clients in needed treatment for addiction and/or mental crisis by providing an evaluation, bed search and support from our team. Behavioral Health Addiction Urgent Care is an exciting, dynamic environment in which all members of the multidisciplinary team work together to ensure the best outcomes for our clients.     Case Managers in Urgent Care coordinate intakes from outside agencies and review referrals to determine if incoming clients are appropriate for our Enhanced Acute Treatment Services, PASSages or Transitional Support Services programs. Case Managers communicate with referring agencies to ensure that clients come to us with everything needed for recovery, including but not limited to documentation and medications.

    Responsibilities

    Essential duties and responsibilities include but are not limited to:

    • Review internal and external referrals for the adult inpatient substance abuse continuum and the Enhanced Addictions Treatment services. 
    • Develop relationships with all referral sources including, but not limited to, emergency rooms, emergency mental health, judicial systems, other substance abuse treatment facilities and other programs within CHL. 
    • Monitor process and flow in the Admissions Department throughout the continuum.  Assist with assessments during admissions, and schedule admissions within the continuum programs.
    • Assist with client admissions by answering the phone, completing all documentation whether on paper or in the electronic record and doing client searches.
    • Assist in creating a safe therapeutic milieu by fostering a culture of recovery through interaction with detox patients.
    • Monitor daily patient activities and provide orientation of clients to the unit.
    • Ensure that clients comply with all treatment groups and unit rules.
    • Demonstrate appropriate and respectful interactions with patients and the community as outlined in agency ethical policies.
    • Effectively foster client compliance with program expectations while delivering culturally sensitive treatment and care.
    • Demonstrate positive communication skills in dealing with clients, visitors, and other members of the health care team.

    Qualifications

     

    • Grade 5: Case Manager I: HSD/GED/HiSet required, Associates degree preferred
    • Grade 6: Case Manager II: Bachelor’s Degree required; degree and/or course work in social work, counseling, psychology or related field preferred.
    • Two years experience in substance abuse or behavioral health field preferred.
    • Current valid US-issued driver’s license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in a personal vehicle
    • Knowledge of the Massachusetts continuum of care for substance abuse.
    • Reliability and flexibility is required.  Day, evening, holiday and weekend work is required.
    • Desire to serve others required.
    • Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives required.
    • Experience working with electronic medical record and Microsoft Excel preferred.
    • Must be able to pass a CORI background check
