    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Case Manager in Worcester.

     

    The HOAP Clinic has provided integrated primary and behavioral health care and substance use treatment for more than 30 years.  Our highly-qualified, trauma-informed providers and clinicians utilize case managers, care consultants, and nurse care managers to provide a person-centered team approach to meet the needs of each patient through individual and group treatment.  The HOAP Clinic has been accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF)  and is a recognized Patient-Centered Medical Home (PCMH) practice.

     

    The Uplift Community Clinic provides respectful, sensitive, and compassionate care to persons who have experienced sexual trauma, commercial sexual exploitation, and sex trafficking. Our services strive to address the physical and mental well-being of each patient who seeks care. Services include access to behavioral health care, general medical care, medication treatment for substance use disorders, access to contraception, and options for case management services. The clinic is co-located in a survivor-led community outreach program and within Community Healthlink’s HOAP Program.

    Responsibilities

    • Complete client-centered case management triage and assessment with individuals who have experienced sexual trauma, commercial sexual exploitation, or sex trafficking.
    • Develop service plans with individuals to address barriers and help survivors reach their goals through the linkage of essential services; clinical, wellness, medical, emergency shelter and/or housing needs.
    • Work with HOAP integrated team and community partners to coordinate the delivery of clinical, medical and other support services.
    • Make referrals to community agencies for additional supportive services as needed within the Worcester community.
    • Provide case management services to assist individuals with gaining access to public benefits (i.e., SSI, EAEDC, MassHealth and other health insurance needs, and other community resources).
    • Provide transportation assistance to individuals as needed. Assist clients in accessing transportation through their health insurance benefits and/or budgeting.
    • Utilize creative and empowering engagement strategies to outreach women who have experienced sexual trauma at identified sites and in the community.
    • Lead and/or co-lead groups and provide group planning support.
    • Participate in case conferences for program participants.
    • Regularly attend staff meetings, team meetings, clinical and administrative supervision and all other required meetings.
    • Complete confidential documentation and service records.
    • Attend and participate in trainings.

    Qualifications

     

    • Grade 5: Case Manager I-High school diploma/GED/HiSet required; Associates degree preferred
    • Grade 6: Case manager II-Bachelor’s Degree required; degree and/or course work in social work, counseling, psychology or related field preferred
    • Lived experience with sexual trauma, commercial sexual exploitation, drug or alcohol misuse/addiction, and/or homelessness.  Must be well along in own personal recovery (self-knowledge and habits needed to support one\'s own health).
    • Experience as a recipient of services for people with sexual trauma, commercial sexual exploitation, drug or alcohol misuse/addiction, and/or homelessness; must be well along in own personal recovery (self-knowledge and habits needed to support one\'s own health).
    • Demonstrated knowledge of victims of sexual trauma, exploitation, trafficking.
    • Demonstrated knowledge of homeless population and relevant community resources.
    • Ability to coordinate all appropriate client services.
    • Ability to effectively communicate with consumers, peers and other professionals.
    • Ability to work within independently and within a group.
    • Timely and accurate documentation skills.
    • Current valid US-issued driver’s license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle and travel between work sites
    • Must be able to Pass a CORI background check
