The HOAP Clinic has provided integrated primary and behavioral health care and substance use treatment for more than 30 years. Our highly-qualified, trauma-informed providers and clinicians utilize case managers, care consultants, and nurse care managers to provide a person-centered team approach to meet the needs of each patient through individual and group treatment. The HOAP Clinic has been accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) and is a recognized Patient-Centered Medical Home (PCMH) practice.
The Uplift Community Clinic provides respectful, sensitive, and compassionate care to persons who have experienced sexual trauma, commercial sexual exploitation, and sex trafficking. Our services strive to address the physical and mental well-being of each patient who seeks care. Services include access to behavioral health care, general medical care, medication treatment for substance use disorders, access to contraception, and options for case management services. The clinic is co-located in a survivor-led community outreach program and within Community Healthlink’s HOAP Program.
