Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) RN-Care Manager in Worcester County.
Community Healthlink’s Behavioral Health Community Partner (BHCP) program is a new, innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) and or a Managed Care Organization (MCO).
Multidisciplinary BHCP teams will play a key role in providing high quality integrated care; functioning as a bridge between clients, behavioral health providers and medical providers. Team members will use their expertise to support individuals to navigate health and social systems in order to help members address social determinants health, and improve overall quality of life and health.
The BHCP RN Care Manager is a core member of the collaborative care team which includes the enrollee’s medical providers and behavioral health providers, and is responsible for supporting and coordinating the mental and physical health care of enrollees assigned. The RN Care Manager works collaboratively with the interdisciplinary team to ensure engaged enrollee’s needs are met and care delivery is coordinated across the continuum.
