    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a part-time (20 hrs/wk) Clinician in Worcester.

     

    Adult Community Clinical Services (ACCS) is a comprehensive clinically focused service anchored by a multi-disciplinary team that provides clinical coverage 24 hours a day, 7 days a week, 365 days a year through an integrated team led by clinical staff.  ACCS serves adults 18 years of age or older with a significant mental health or substance abuse diagnosis.  ACCS Integrated Teams provide clinical interventions, employment support and peer and family support to facilitate engagement, support functioning and maximize symptom stabilization and self-management of individuals residing in all housing settings.   The Persons served through ACCS reside in group living environments, supported individual environments, Intensive clinical or Intensive Medical group living environments or in independent living environments in the community.

     

    The role of the clinician is to deliver clinical services to adults with substance use and mental health issues and provide clinical oversight as a member of an innovative multi-disciplinary team.

    Responsibilities

    • Conduct evaluations including bio-psychosocial assessments, diagnostic formulation and treatment recommendations.
    • Facilitate rehabilitation groups using standard group techniques. Meet with persons for individual counseling to assist in achieving treatment goals.
    • Write treatment plans including person's strengths, abilities and preferences with behaviorally based goals and objectives. Document progress of goals and objectives in progress notes.
    • Follow a person-centered model and perform person-centered planning (IAP's), working with severe and persistent mental illness and dually diagnosed clients.
    • Attend all scheduled meetings on time and remain for the duration of the meeting.
    • Complete MAP certification within 5 months of employment
    • Be an active participant in the multi-disciplinary team meeting and case discussions.
    • Recognize and seek consultation as needed. Develop a training plan to enhance professional growth.
    • Refer consumers to appropriate resources.
    • Follow all policies and procedures of the agency.
    • Participate in utilization management, employ CQI principals and assist as needed in accreditation processes.
    • Position may include some evenings and weekends on-call.
    • Position may require moderate physical duties.

    Qualifications

     

    • Grade 7: Clinician III: Master's Degree in social work, counseling with a MA license to practice is required *or must be licensed within one year of hire. Acceptable licenses include Licensed Certified Social Workers (LCSW), Licensed Mental Health Counselor (LMHC). Above credentials from another state that meets MA requirements is acceptable with immediate application for transfer.
    • Grade 8: Clinician IV: Masters degree in social work and a MA LICSW.
    • Current valid US-issued driver’s license and ability to provide registered, inspected and insured vehicle for work related purposes including transporting clients in your own vehicle
    • Bilingual English/Spanish preferred
    • Must be able to pass CORI background check
