    Come join a friendly and diverse team of professionals who provide quality home visiting services for infants and toddlers. Community Healthlink (CHL) is looking for Physical Therapists who are interested in working a few hours per week during the moring, early evening and/or weekend hours on a fee-for-service basis in the North Worcester County area.


    The Early Intervention (EI) team provides strength-based, family-centered services for young children under three years of age who have developmental delays or are at risk of a developmental delay.  Services are designed to meet the needs of each infant or toddler, and the needs of their family. The EI team provides services in the home/community setting with the goal of enhancing the child’s physical, cognitive, communication, social-emotional and adaptive skills. 


    • Provide direct intervention services to children 0-3 and their families.
    • Conduct evaluations and assessments particular to individual specialty.
    • Develop and implement Individualized Family Service Plans.
    • Coordinate services and act as a resource for children and families.
    • Assist in transition planning prior to discharge from the program.
    • Attend staff meetings, in-service education and other professional meetings as required.
    • Maintain client and administrative records according to center policy.
    • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.



    • Doctorate or Masters Degree
    • MA PT license required. 
    • Minimum of 1 years experience working with young children and families

    General Qualifications:

    • Current valid US-issued drivers license and a registered, inspected and insured automobile for work related purposes including travel between work sites
    • Excellent interpersonal, verbal, written and communication skills.

    • Flexibility; ability to work independently and contribute to overall success of team.
    • Bilingual (English/Spanish) candidates are encouraged to apply.
    • Current valid US-issued drivers license and ability to provide a registered and insured automobile for work related purposes, including travel between work sites 
    • Must be able to pass a CORI background check
