    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a Full-time (37.5 hrs/wk) Director of Housing in Worcester.

     

    Adult Community Clinical Services (ACCS) is a comprehensive clinically focused service anchored by a multi-disciplinary team that provides clinical coverage 24 hours a day, 7 days a week, 365 days a year through an integrated team led by clinical staff.  ACCS serves adults 18 years of age or older with a significant mental health or substance abuse diagnosis.  ACCS Integrated Teams provide clinical interventions, employment support and peer and family support to facilitate engagement, support functioning and maximize symptom stabilization and self-management of individuals residing in all housing settings.   The Persons served through ACCS reside in group living environments, supported individual environments, Intensive clinical or Intensive Medical group living environments or in independent living environments in the community.

    Responsibilities

     

    • Performs duties using recovery orientated principles.
    • Ability to supervise Site Managers, Housing Coordinators and other staff to ensure quality clinical programming.
    • Effective communication with service providers to ensure quality clinical practices.
    • Demonstrate person-centered and strength-based communication and relationship with Persons.
    • Demonstrate effective communication with staff, peers and other professionals.
    • Maintain and comply with all regulations required for operations; i.e.: DMH Licensing/Rehab Option, DDS, (MAP) Medication Administration Program, HUD, DPH, and WHA. Maintain CPR and First Aid certification.
    • Quality Management: Provide oversight of specified program components. Identify and participate in quality improvement activities.
    • Budget Oversight: Ensure operational budget for the programs is reconciled. To supervise census and special billing monthly and to track movement of Persons for census/billing purposes.
    • Supervision and Training: Orient new site supervisors to daily operations of the program and Division procedures. Ensure new staff are oriented and trained as required by CHL. Provide ongoing supervision to Site Managers and other staff as designated.
    • Referrals: Oversee Persons admissions, transitions, and discharges. Strive to maintain maximum capacity through outreach and coordination with other providers
    • Program Management: Evaluate staffing patterns and coverage, services on-site, and rehabilitation progress in order to ensure safety and security of residents.
    • Personnel: Take personnel action in regard to hiring and disciplinary procedures
    • On-Call: Participate in administrative on-call through the emergency on-call system.
    • Liaison: Serve as liaison among other Divisions and with other agencies in order to facilitate a continuum of care.
    • Facilities: Identify problem areas and coordinate work for upkeep of sites.
    • Housing: Provide oversight to Housing Coordinators and staff as appropriate to ensure appropriate Housing Related Services and/or oversee support to Persons to access and maintain independent housing

    Qualifications

    • Master’s degree in social work or related human services field or Bachelor’s degree plus four years of experience in Human Service Management
    • Current valid US-issued driver’s license and ability to provide registered, inspected and insured vehicle for work related purposes including transporting clients in your own vehicle
    • Bilingual English/Spanish preferred
    • Must be able to pass CORI background check
