    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Care Coordinator Case Manager in Leominster.


    Community Healthlink’s Behavioral Health Community Partner (BHCP) program is a new, integrative case management program that offers support to MassHealth members with complex medical, behavioral health, substance use treatment and social service needs.  This innovative program is a whole-person health approach and CHL is looking for individuals interested in learning more about this statewide initiative and how to offer real, collaborative case management care.   


    Care Coordinators will work with multidisciplinary teams of RN’s, licensed behavioral health clinicians, outreach specialists and fellow case managers to offer MassHealth members an integrated care team approach to support their overall health needs.

    • Participation in on-boarding training in Community Health Worker training
    • Learn how to create and manage Person Centered Treatment Planning
    • Work with an integrated care team
    • Meet members in the community or office settings to complete Comprehensive Assessments
    • Help members identify their overall medical, behavioral and social service needs
    • Learn to use Motivational Interviewing and recovery oriented principles with  members
    • Help member’s set wellness goals and promote healthy activities
    • Learn how to provide clear and timely documentation in a care management platform
    • Complete all paperwork as defined by CHL’s guidelines
    • Perform other job related job duties


    • Grade 5: Case Manager I: Minimum of High School Diploma/GED or a HiSet required and a Community Health Worker Certificate; Associates degree preffered
    • Grade 6: Case Manager II: Bachelor of Arts or Science in a related field
    • Must have a minimum of 2 years recent experience
    • Organization skills and the ability to work cooperatively on a team
    • Current valid US-issued drivers license and ability to provide registered, inspected and insured automobile for work with potential transportation of client in your own vehicle
    • Must be able to pass a CORI background check
