    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (40 hrs/wk) Case Manager in Worcester, MA.

     

    The Program for Assertive Community Treatment, or PACT, is an intensive and highly integrated evidence based practice for community mental health services. Our team serves 80 individuals age 18 + who struggle with serious and persistent mental illness and whose symptoms result in serious functioning difficulties in several major areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.

     

    Unlike other community-based programs, PACT is not a linkage case-management program that connects individual’s services. Rather, it provides highly individualized services directly to clients. PACT is multi-disciplinary team model who whose goals are to support clients to reduce hospitalizations and to restore and enhance the emotional well-being and level of social functioning of clients. Services are provided in clients’ own homes and communities and are provided 24 hours a day, seven days a week, and 365 days a year. This program serves clients in Worcester and its surrounding areas.

    Responsibilities

    • Work as part of a multi-disciplinary serving people with severe and persistent mental health issues.
    • Familiar with evidenced based practices including harm reduction, motivational interviewing, etc.
    • Ability to assess basic physical, social, educational, legal needs.
    • Required to transport clients to appointments, assist with ADL support, vocational and educational support and community integration.
    • Staff is responsible for developing and implementing treatment plans for clients every six months and completing notes daily.
    • The case manager will be expected to maintain a caseload and assist with observing medications and providing budgeting support.
    • Familiar with principles of mental health and addiction recovery and able to offer a hopeful coaching approach to support continuing recovery of the people served.
    • Attend program staff team meetings, supervision, trainings and all other required program meetings.
    • Complete documentation in a timely and thorough manner.

     

    This position has a Tuesday to Saturday schedule

     

     

    Qualifications

    • Grade 6: Case Manager II: Bachelor's Degree in psychology or related field, preferably with 1 to 2 years experience
    • Must have a strong commitment to the right and ability of each person with a severe mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.
    • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle or a company vehicle
    • Electronic medical record experience is a plus
    • Ability to demonstrate clear boundaries with clients while being client centered and supportive
