    Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Advanced Practice Psychiatric RN in Worcester.


    The Geriatric Team provides behavioral health services, including psychiatric consultation, to nursing homes in Central Massachusetts and MetroWest. Staff include licensed independent clinical social workers, licensed mental health counselors, psychiatrists, advanced practice psychiatric nurses, and administrative support staff.

    • Provides assessment, diagnosis, medication consultation, and follow-up services for individual adults in nursing homes in Central Massachusetts.
    • Provides evaluation and consultation to prescriber's, nursing staff and families.
    • The APRN and psychiatric clinicians provide recommendations for psychiatric medications.
    • In collaboration with the clinical team this includes strengths, goals, need areas and additional consultation and collaboration as needed for treatment and safety
    • Works closely with a psychiatric supervisor employed by UMass Memorial Healthcare through Community Healthlink.
    • Provides consultation services at nursing home facilities throughout Central Massachusetts and MetroWest.
    • Works collaboratively as a member of patient-centered treatment teams, including nursing home staff, to coordinate the delivery of clinical and medical services.
    • Maintains all documentation for services provided, with review of information from other providers in the EMR.
    • Attends relevant training activities.
    • Regularly attends required program meetings.
    • Prepares and maintains confidential client documentation, service records, service activity logs (SALS.)
    • Performs other related job duties as required

    • Licensure as a Psychiatric Clinical Nurse Specialist or Psychiatric Nurse Practitioner by the Massachusetts Board of Registration in Nursing
    • Current DPH and DEA certificates
    • Meets all qualifications for the advanced practice of psychiatric nursing with prescribing privileges
    • Experience working with adults with severe and persistent mental illness, substance use disorders, developmental disabilities, and co-occurring developmental disabilities and/or medical conditions
    • Demonstrated knowledge base of adults requiring nursing home care
    • Demonstrated current knowledge of psychiatric medications
    • Ability to work as a member of a team
    • Ability to accurately assess client’s presenting concerns
    • Ability to effectively communicate with clients, peers, and other professionals
    • Timely and accurate documentation skills 
    • Must be able to pass a CORI background check
    • Current valid US-issued drivers license and ability to provide a registered and insured automobile for work related purposes including travel between work sites
