HOAP - Homeless Outreach & Advocacy Project:  Many years ago Community Healthlink recognized that so many of the homeless in our society also battle mental illness, and often a substance abuse problem. In 1985, the Homeless Outreach & Advocacy Project was established with a commitment to link the region's homeless to a complement of services that would offer: screenings, assessments, counseling, health care, rehabilitation, and referral to other support systems.  In addition, a physician and registered nurses and nurse practitioners perform medical evaluations at HOAP's primary care clinic, 72 Jaques Ave., Worcester. They visit area shelters to evaluate and refer people for treatment. A psychiatrist and a team of licensed clinicians perform mental health screenings and offer counseling and medications. Case Management staff helps clients to enroll in available health insurance programs, access health care and social security benefits, and resolves other benefits-related issues.

 

Outreach performed by HOAP staff is done in the community, through a variety of shelters and other emergency care programs. Staff also seeks to establish contact with the homeless at vacant lots, and even along railroad tracks and bridges. Basic services are also available at HOAP's primary care clinic and staff offices.

 

HOAP also offers services in partnership with other local health care providers such as the Great Brook Valley Health Center and Family Health Center in Worcester.

Outpatient Behavioral Health and Primary Care Services: Our Leominster Counseling Center Outpatient Behavioral Health and Primary Care Services specialize in caring and professional services for adults with serious mental illness and co-occurring disorders. Outpatient care might include individual, family or group psychotherapy, psychiatric services, medication management, wellness services, crisis management, or case management.

 

The Outpatient Team includes psychiatrists, nurses, social workers, psychologists and mental health counselors trained and experienced in the care of those with serious mental health conditions. We work closely with primary care physicians, area hospitals and inpatient programs, and the Department of Mental Health to coordinate care. Services are available in Spanish, and English.

 

Community Healthlink’s Counseling Centers feature services for children, adolescents, adults, couples and families. Our clinics are located in Worcester, Clinton, Fitchburg, Leominster, and Gardner.

CHL's PACT (Program for Assertive Community Treatment) Program is a DMH closed referral program that provides intensive treatment and support services for adults who have been diagnosed with persistent chronic mental illnesses. The program utilizes a multidisciplinary team that includes a psychiatrist, registered nurses, social workers and vocational specialists whose goals are to support clients to reduce hospitalizations and to restore and enhance the emotional well-being and level of social functioning of clients.  The program also works closely with clients to find housing and to provide vocational training and employment opportunities.  The PACT program works with individuals residing in Worcester.

The Program for Assertive community treatment, or PACT, is an intensive and highly integrated approach for community mental health service delivery that serves individuals who struggle with serious and persistent mental illness and whose symptoms result in serious functioning difficulties in several major areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.

 

Unlike other community-based programs, PACT is not a linkage case-management program that connects individual’s services. Rather, it provides highly individualized services directly to consumers. PACT is organized in a team model that includes doctors of psychiatry, mental health therapists, nurses, and vocational specialists who work together to create individualized plans for clients and provide the care needed to meet the goals of these plans. Services are provided in clients’ own homes and communities and are provided 24 hours a day, seven days a week, and 365 days a year.

 

Community Healthlink operates two PACT teams in the Worcester County area. One covers the North Worcester County area (PACT III) and the other cover the Central Worcester County area (PACT). This program is closed to only DMH referrals.

Crisis Stabilization Unit (CSU)

Community Healthlink's Crisis Stabilization Units are located in Worcester and Leominster. These community-based units provide short-term therapeutic psychiatric treatment in a structured environment with 24-hour observation and supervision and individualized treatment interventions focused on the crisis and crisis resolution. The CSU accepts referrals from UMass Emergency Mental Health Services (EMH), 8 East, PTC, and CHL’s own Emergency Services Program, as well as other area inpatient facilities or emergency departments with an evaluation and client history completed by a qualified clinician prior to admission.

 

The CSU may be used as a diversion to prevent a hospitalization or as a step-down immediately following a psychiatric hospital discharge to assist the client in transitioning back to the community. The unit receives admissions 24 hours a day, 365 days a year, depending on bed availability for clients who do not need hospital level of care and meet the unit's admission criteria. The population includes both men and women who are at least 18 years of age.
