The program provides individual comprehensive case management services to each client along with at least four treatment groups per day, run by all team members (residential counselors/recovery specialists, case managers, clinicians, nurses).    Detox also has  an EATS Program ( Enhanced Acute Treatment Services) within this level of care that supports individuals also dealing with active sysmptoms of their mental illness. Services include daily individual counseling as well as a psychiatric consultation with an MD or NP.    Detox is an exciting, dynamic environment in which all members of the multidisciplinary team work together to ensure the best outcomes for our clients. The OBOT program offers buprenorphine and vivitrol treatment.   The OBOT Nurse meets with clients for the purpose of medication evaluation, assessment and treatment planning. The CSU may be used as a diversion to prevent a hospitalization or as a step-down immediately following a psychiatric hospital discharge to assist the client in transitioning back into the community. The unit receives admissions 24 hours/day, 365 days/year depending on bed availability (13 beds) for clients who do not need hospital level of care and meet the unit’s admission criteria. Although a voluntary program, individuals must remain on the unit for the duration of their stay for safety and treatment purposes. Clients meet with a multi-disciplinary team to aid with following their individualized treatment plan and meet their treatment goals as well as a psychiatric workup and medication evaluation.   Community RN's provide   nursing   assessment,   treatment, education and health care service coordination to individuals  and families. Their focus is on promoting and maintaining an optimal level of health. The RN collaborates with all components of the team and other health care agencies to ensure coordinated, comprehensive services. Our team serves up to 50 individuals age 18 + who struggle with serious and persistent mental illness and whose symptoms result in serious functioning difficulties in several major areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.   Unlike other community-based programs, PACT is not a linkage case-management program that connects individual’s services. Rather, it provides highly individualized services directly to clients. PACT is multi-disciplinary team model who whose goals are to support clients to reduce hospitalizations and to restore and enhance the emotional well-being and level of social functioning of clients. Services are provided in clients’ own homes and communities and are provided 24 hours a day, seven days a week, and 365 days a year.
