Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Milieu Manager at our Worcester site.   Faith House is a 25-bed six-month residential treatment program that serves women in recovery from substance use disorder. The program provides a variety of rehabilitative services where a team of staff implement treatment plans based on the specific needs of persons served. Community Healthlink (CHL) is looking for a Full-time (37.5 hrs/wk) Program Coordinator in Worcester.   Massasoit Rd is a one floor residential treatment program serving eight adult individuals struggling with persistent mental illness who potentially have co-occurring medical conditions or are mobility-impaired. The program provides a variety of services where a team of staff implement treatment plans based on the specific needs of persons served. Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Program Coordinator for Homeless Supportive Housing in Worcester.   Community Healthlink’s Homeless Outreach and Advocacy Project (HOAP) provides direct services to individuals who are homeless. Services include outpatient primary health care, behavioral health care (individual and group mental health and addiction treatment), and outreach and case management services. Working together, our integrated, multi-disciplinary treatment teams address physical and mental health needs, substance use disorders, and identified social determinants of health by providing access to a wide-variety of patient-centered, trauma-informed treatment and services. CHL HOAP is a Federally Qualified Health Center (FQHC) Health Care for the Homeless (HCH) program.   The HOAP Housing Programs provide intensive case management to homeless individuals and families.
